Established in 1998, the American Medical Association Foundation serves as an educational resource and network for physicians across the country. In addition to cultivating industry leaders, the organization works to develop and disseminate best healthcare practices and models nationwide.

To that end, the foundation is accepting applications to its Community Health Program. Through the program, grants of up to $60,000 will be awarded to community-based organizations with programs focused on the prevention and reduction of type 2 diabetes and hypertension. Priority will be given to programs that employ evidence-based practices and serve vulnerable or hard-to-reach populations. Grants are renewable for up to three years.

To be eligible, organizations must be tax exempt under section 501(c)(3) of the Internal Revenue Code; have been in operation for at least three years; have an annual operating budget of $3 million or less; and provide services within the continental U.S.

A non-mandatory, informational webinar detailing the opportunity will be hosted by the foundation on February 12, 2019.
